Start with increasing your self-efficacy!

Do you, or someone you know, suffer from chronic pain?

If not, then you are most lucky – or you don’t know the people around you too well. No offense, and this is not cynicism speaking – this is numbers talking, and cultural dispositions not to talk about certain things like, let’s say women’s periods. Did you know that 1 out of 10 women between teenage years and mid-fifties is struggling with Endometriosis? That is not a rare disease – and we are looking at plenty of women where the diagnosis has yet to be taken, because they were made-belief that period pain were normal. When looking at such a thing like Endometriosis we are talking about pain that is not just uterus cramps – but back pain, bladder pain, fatigue symptoms, nausea, and many more (1). If you do not know about it, don’t believe it or feel uncomfortable right now (maybe even thinking of skipping this article), then maybe because we are not used to speak about women’s periods in the work environment. So let’s move away from this non-topic: Numbers get even worse when we look at migraine. More than 1 billion people globally are suffering from migraines, in Austria alone we’re more than 1 million (ouf of 9 million people living in Austria in total) (2). And this is just two examples – we could go on.

However, this article is not about ranting.

Instead, let’s take a step back: How do all these people, how do you balance chronic pain with a demanding professional setting? And let’s be honest: It IS a struggle. Just because many people are not even aware it exists does not mean it’s not the reality of many. Some days ago, I had a chance to give a short impulse on „Self-efficacy in a multi-cultural work environment when balancing chronic pain with a demanding professional setting“, at the United Nations Vienna office, in a charity and awareness event for Endometriosis Österreich, organized by Guiseppe de Simone (United Nations) and Verena Anna Buck (https://endometriose-oesterreich.at/en/). My preparation for this sleek 10 minute speaking slot made me think: What is the one thing that you can, that maybe you have to start with? And when thinking multi-cultural, which certainly is a topic for the UN, and in the meantime also for many companies and corporations globally, with their international set-up and people from different origins working hand in hand – how do we actually tackle it, when there is so many cultural differences to consider and respect?

I felt, we need to step back and start, as usual, with ourselves. So, thinking and talking about self-efficacy can sound – again – cynical, when we speak about chronic pain, and I understand why someone with chronic pain might now roll their eyes at me – please bear with me though! And, in addition, let’s be very specific about one thing here: Everything in this article is about balancing the chronic pain with a demanding work set-up. This excludes people that are bedridden, or people who cannot even make it to work anymore. We need to be this specific here: When including everyone with chronic pain, the level of complexity is rising to such an extent, that I believe I cannot even say any more what I want to share as a train of thought.

So, back to the people (you?) that are still able to keep it up, that still show up at work (at least most of the time), that still try to live a life as „normal“ as possible (I know, there is no „normal“ – just for lack of a better word).

In all the complexity and overwhelm of regular business life – how do you manage to not end up a wreck?

How can you hold still for a second, to understand the details of what is going on with you in that very moment?

Let’s take a minute. These are a few questions to reflect upon, you could even take a few notes if you wanted to. We start with the outside – with the work place, your daily work environment. Let’s collect some data by challenging your own assumptions and beliefs on who you are, or who you ought to be. Take these questions one by one, and only continue once you finished your reflection on each of them. If now is not the best time – get back to them later, and continue below, if that feels better right now.

What means: Demanding?

What is the actual demand, and the expectation of others in this moment, and what is your own demand and expectation of yourself?

How do you identify in the workplace? What is your belief about yourself?
Are you a „hard worker“? Someone that „always pushes through“?
Is dealing with chronic pain „a weakness“? That „doesn’t allow you“ to take on interesting things?

By understanding your own evaluation of the situation, and the meaning and therefore judgement you put behind it, you already get one step closer to getting back into the driver’s seat. If you are not the one with the chronic pain – this could still be interesting to think about. Because even though you might not be aware, chances are more than high that you’re dealing with someone in deep pain today, tomorrow, and next week.

Now, challenge your beliefs on how you need to deliver at work, and what are your expectations versus other’s expectations towards you. Pick one specific situation that you can remember, where you were in pain and at the same time you felt you need to deliver, or you couldn’t deliver:

Where is your focus in this specific situation? What did you maybe ignore or didn’t pay attention to? Are there other sources of „data“ that you didn’t consider?

How do you interpret the situation? Are you looking at it objectively? What would someone else see, if they took a closer look? What other meanings could something have that has been said (or not)?

Based on those interpretations – what assumptions did you take? Are they valid?
Why are you assuming this?

Which conclusions do you draw out of it? Why did you conclude this?

And finally, which beliefs do you hold about it? What conclusions are they based on? What action do they typically lead to? (3)

When you get ahold of the beliefs you are carrying with, be it about yourself or about others, you already took a mighty first step. Our beliefs, that are feeling somewhat imprinted in us, often are coming from way back, from our families, the ecosystem that brought us up, our school experiences, our first work experiences. They probably helped us in many great ways to get to where we are at – and yet some of them certainly are not being useful any more. So getting a better understanding of which beliefs you hold about yourself (and therefore also others probably) is great to start challenging them.

So, now let’s try to look at it differently. If you were your best friend, or if the person pushing through too much, or not pushing enough, if they were your very best friend, a person you truly love and care for – what would you recommend them to do?
Often, people have a tendency to be harsher with themselves as they would be with a precious friend. What would you tell someone you fully appreciate in this moment, when they feel the demand and expectation, and need to figure out if it is their own, or someone else’s?

Doing this, we are on a continuous track of increasing self-awareness – which is the superpower for leaders of all kind, for self-development – and for people dealing with chronic pain. And again, please do not read this with a voice of cynicism. The point I am trying to make, is: It is not always helpful when you dealing with chronic pain to ignore to what you need right now (to be fair: sometimes, when dealing with chronic pain, it is also not helpful to listen too closely to what you need right now). What is helpful is: To make it a conscious decision. If you listen to your needs, or if you are deciding to skip them. To allow for space for the pain, and what your body requires, or to disassociate from the pain and keep going.

How to make it a conscious decision?

Start with regular body check-ins. Not just in the moments of pain, but make it a regular thing.
To understand and to be fully aware of your physical need is an important and useful information, and thus data you want to include in your decision making process.

Next thing: Try to figure out the different voices in your head, or the different parts you have in a certain moment. What I mean by that is: We all know to be torn in certain situations. Like, I understand why my friend didn’t ask me to join for their partner’s birthday dinner, AND at the same time it hurts and feels wrong. Or one side of us shares the perspective of the colleague notoriously complaining about the workload, and yet another side of us also completely understands why the whole team is putting in the hours these days, and wants to be a part of it, and even enjoys the hustling. I am sure you can come up with plenty of situations of your own where you felt a dilemma and could share and agree to more than one position. Now, this happens also inside us when it is not about the actions of others, but our own behavior and activity and maneuver. When talking about chronic pain, that can mean: You have this one side that really feels you need to take care of yourself and just lie down, and there might be the other side that also feels that you really want to push through with the presentation, or the meeting, or whatever you are doing right now for a very good reason. This is a real thing – to want more than one thing at the same time, and to deal with this inner conflict, dilemma and whatsoever you’re going through with yourself, it happens to most people most of the time.

Why this matters when dealing with chronic pain? To understand what are your different inner sides or parts in that very situation, to understand your own needs, and then to make it a conscious decision on which of the competing needs you are going to act on – THAT is self-efficacy. It does not lower the pain – but it might help with the dissociation, if you decide to pull through consciously rather than by default. And it also helps with the frustrations of not being able to do everything you wanted to do in the time you need to take to deal with the pain, to rest, or just to survive it (when you are not the one dealing with chronic pain, this might sound dramatic – please believe me, it is not. Depending on what you are facing, this can be very real).

Why self-efficacy matters

All of this might not sound like much. If you’ve been there, you know what I am talking about, or trying to say. It is so unbelievably hard to deal with chronic pain, to know it will come again in a few days, or not to know when it creeps in again. It is so hard to go through the pain, again, and again, and again, knowing, there is probably not an end to it, which is true for many forms of chronic pain (though gratefully for some of them there is medication, or surgery, or hormones, or other stuff that again probably has side-effects you need to deal with…). It is so hard to go for a career, when you do not even know how much to trust your own body, or when it might let you down, or however it makes you feel. And it is hard to deal with the structural disadvantages of being someone who physically cannot deliver all the time at all cost (as if there was such a person who could do that 😉 ).

When you do not want to be the victim, when you need to be the one who is still in the driver’s seat, rather than the chronic pain taking over your life (and the ones of the people you love and share this life with), choosing self-efficacy and making it a point to decide on how to deal with it today, in this moment – this is what can keep you going. And it will help you to break a cycle of beliefs that maybe are not always too helpful (you can keep the ones you still like though). There is a million studies on why self-efficacy matters – increasing resilience, influencing goals you choose for yourself, the way how you act, feel and think. To make your behavior your own decision when dealing with chronic pain – that means you’re taking the power back from the chronic pain.

If you believe this is an interesting topic, or you want to learn more about how to do this, don’t be shy to reach out.

Footnotes:

(1) Find loads of info on this page: https://endometriose-oesterreich.at/en/

(2) https://www.meduniwien.ac.at/web/ueber-uns/news/2024/news-im-mai-2024/eine-million-menschen-in-oesterreich-von-migraene-betroffen/

(3) Chris Argyris, in: Peter Senge (1990), The Fifth Discipline: the art and practice of the learning organization.
